The right CHM program for you

Is your CHM program the one that best meets your needs? If not, please don’t wait to switch. You can change your program by calling the CHM Member Assistance dept. at 330-848-1511, extension 5993. (Please note that if you switch while experiencing signs, symptoms, treatment or testing of a condition, your medical bills will be authorized at the lower participation level.)

Gold program (65 percent of CHM members are Gold)
$150 per membership unit, per month

The Gold program provides members with the ministry’s most extensive financial support and offers many advantages not included with Silver or Bronze.

At Gold, CHM shares 100 percent of bills for any medical incident exceeding $500 as long as all other Guidelines are met. (Total bills incurred per incident must exceed $500 in order to be eligible for sharing.) You can receive assistance up to $125,000 per illness.

Example: You receive treatment early in the year for a gallbladder problem. Your bills total $7,500 and you receive $2,500 in healthcare provider discounts. CHM shares $5,000 ($7,500-$2,500), which is 100 percent of the cost after discounts. In the middle of the same year you have some blood tests done (for an unrelated illness) that cost $400. CHM does not share these bills because the total cost of the incident is less than $500. At the end of the year, you break your arm and treatment amounts to $2,500. CHM shares the entire $2,500 because the incident totals more than $500 and the treatment meets all other CHM Guidelines.

Gold plus Brother’s Keeper: Joining at Gold and becoming a Brother’s Keeper member provides unlimited financial assistance for all eligible medical bills.*

We recommend Gold to all members, but especially if:

  • You’re a woman who may become pregnant.
  • You have a personal or family history of health issues.
  • You have one or more pre-existing conditions.
  • Spending more than $500 out-of-pocket for healthcare costs doesn’t work with your budget.

 

Silver program (15 percent of CHM members are Silver)
$85 per unit, per month

The Silver program includes only inpatient or outpatient hospital incidents and procedures performed in a certified surgery center. Testing and treatment performed outside a hospital are not eligible for sharing.

Silver has a $1,000 personal responsibility per incident. You will pay $1,000 (or receive at least $1,000 worth of discounts on your medical bills) per medical incident before CHM helps share your hospital or surgery expenses. Assistance is up to $125,000 per illness.

Silver plus Brother’s Keeper: Joining Brother’s Keeper provides an additional $100,000 of cost support.* With each annual Brother’s Keeper renewal, members receive an additional $100,000, accruing up to $1 million per illness.

We recommend Silver if:

  • You’re comfortable spending $1,000 out-of-pocket for any medical incident that arises (CHM steps in to help after you have met your $1,000 personal responsibility).
  • You’re comfortable paying for all treatment that occurs outside a hospital and all prescriptions.

 

Bronze program (20 percent of CHM members are Bronze)
$45 per unit, per month

The Bronze program includes only inpatient or outpatient hospital incidents and surgery performed in a certified surgery center. Testing and treatment performed outside a hospital are not eligible for sharing.

Bronze has a $5,000 personal responsibility per incident. You’ll pay $5,000 (or receive at least $5,000 worth of discounts on your medical bills) per medical incident before CHM helps share your hospital or surgery expenses. Assistance is up to $125,000 per illness.

Bronze plus Brother’s Keeper: Joining Brother’s Keeper provides an additional $100,000 of cost support.* With each annual Brother’s Keeper renewal, members receive an additional $100,000, accruing up to $1 million per illness.

We recommend Bronze only if:

  • You’re comfortable spending $5,000 out-of-pocket for any medical incident that arises (CHM steps in to help after you have met your $5,000 personal responsibility).
  • You understand that bills cannot be shared if services occur outside a hospital.

 

*Editor’s note: If you join Brother’s Keeper while experiencing signs, symptoms, treatment or testing of a condition, bills exceeding the $125,000 limit for that illness cannot be shared by Brother’s Keeper.
