Sharing Request Packet
The Sharing Request Packet is required for CHM to process and share medical bills and must be submitted with each new medical incident.
To submit medical bills online, log in to your Member Portal account.
Please note: Add-on bills to a prior incident do not require new forms.
CHM Give Request
The CHM Give (formerly known as Prayer Page) Request Form is required if you have a pre-existing condition, as defined by the CHM Guidelines.
There are several ways that you can support your fellow Christians through CHM Give:
- Pray as you go through the list of members with eligible maintained pre-existing conditions. We believe in the power of prayer to change hearts and lives.
- Donate through the Member Portal. You can easily set up recurring donations or make a one-time donation through the your Member Portal account.
- If you don’t have a Member Portal account, you can donate by visiting our streamlined CHM Give webpage.
- Send cards, letters, emails, or notes of encouragement to members as you feel led by the Holy Spirit. You never know who you might impact through your act of service and kindness.
