As members of Christian Healthcare Ministries (CHM), you’re empowered to take charge of your health, enabling you to choose the best healthcare providers for your needs, interact with your providers, get the treatment you need without interference, and more.
It also means you’re responsible for submitting your own medical bills to CHM. If you’re unsure how it works, we’ve got your back with this easy-to-use guide on CHM’s medical bill submission process.
Request your itemized bills
The submission process starts when you request itemized bills from your providers. Itemized bills contain the following information:
- Provider information
- Patient name
- Description of service (or CPT code)
- Date of service
- Total charges
Medical bills must be submitted to CHM within six months of the date of service. Bills are processed in the order they are received. The sharing clock starts as soon as you submit your medical bills, so submit your medical bills as soon as possible.
Submit medical bills via the Member Portal
Along with your itemized medical bills, you’ll also submit a Sharing Request Packet. The packet includes the following forms:
- Sharing Request Form
- Medical Bill Worksheet
- Medical Information Release Form
- Maternity Verification Form (only for maternity submissions)
- Prayer Page Form (only for maintained pre-existing conditions)
The Member Portal is the easiest and most efficient way to submit your medical bills and forms. The Sharing Request Packet can be downloaded from the portal, but you can also fill out your forms digitally.
After you complete the questions, you’ll have the option to submit your medical bills quickly and easily by uploading them onto the portal. When you’ve uploaded your information and medical bills, you can access your upload history—and even keep track of where your bills are in the sharing process.
CHM staff will also communicate directly with you through the Member Portal; we’ll use portal notifications to send requests for missing itemized bills, medical records, or forms.

The Member Portal also gives you access to important membership resources such as the CHM Guidelines. All sharing eligibility decisions are made based on the Guidelines, so please stay current on important eligibility information that impacts you. Through the CHM Guidelines, you can know what’s eligible for sharing before you even submit a bill.
